For those members who belong to Central States, you can request information on your Health & Welfare and Pension by calling Central States at 1-800-323-5000, or visit them at the website link on this page. When you sign up as a participant on the website you will have full access to all of your claims, forms, pension information, what plan you are in, what services you have and the type of coverage information, and this will ensure your privacy under the HIPAA Laws. If you do not receive satisfaction from them get a contact name of who you spoke to and call Yvonna Smith at Local 41 at 816-924-2000 Ext. 117; she will try to help you rectify the problem.
For pension applications please call and make an appointment with Yvonna Smith before coming to the office so that she will be able to allot the time needed to process your application and to ensure that she is not out of the office. We know that some of you live far away and don't want you to make the drive to find out she is not there that day.
